As announced to departments on 25 January 2013, the GS Forms 14, 19, and 20 have been replaced with the new GS Form 32 effective 4 February 2013. However, to ease the transition, the Thesis/Dissertation Office will continue to accept forms 14, 19, and 20 presented by candidates at their final deposit appointments until 1 March 2013 as long as the forms are correctly completed and signed. Please note that candidates will not need to attach the GS Form 32 to the front of their Electronic Thesis Deposit documents-just the ETD Form 9 linked below. The GS Form 9 and ETD Form 9 forms have not changed and they will remain in effect. Sections of this website pertaining to the new GS Form 32 will be updated as soon as possible. Questions?
